Online reporting of adverse reactions

Please note:

You have 20 minutes to report an adverse reaction before the session is automatically closed and your entered data in the webform is deleted. Each time you press a button, the timer will start running again at 20 minutes. After sending a notification, the data entered are added to the database and are automatically deleted from the web form for security reasons.
